TEST INSTRUCTIONS

For guaranteed confidentiality and anonymity, follow the "Confidentiality and Anonymity Instructions" contained in this document.

Please follow the instructions below to ensure your samples are collected properly.  We recommend you read the instructions through entirely one time before beginning your collection.  Only collect one sample at a time.

Pre-Collection Item Checklist

For Forensic Sample

1 plain, white envelope or large piece of clean, plain, white paper for each semen-stained sample to be submitted
 
  1 plastic bag for each semen-stained sample to be submitted
 

For Comparison Sample

Depending on the type of comparison sample you submit, the pre-collection checklist will differ.

If using swabs as the comparison sample:
 

2 cotton-tip sterile swab or regular cotton-tip swab (e.g., a Q-Tip) per person tested.

IMPORTANT:  If you use a regular cotton-tip swab such as a Q-Tip, cut off one of the cotton-tips so the swab only has one cotton-tip.  DO NOT TOUCH THE REMAINING COTTON TIP WITH YOUR FINGERS.

Sterile cotton-tip swabs can be found at your local pharmacy behind the counter and are approximately $0.25 per pack (2 swabs per pack).

2 small paper envelopes per person tested
 
1 small plastic bag (e.g., Ziploc bag) per person tested
 
1 Sample ID Slip per person tested (located at the end of this document)
 

If using hair as the comparison sample:

Tweezers (cleaned with rubbing alcohol)
 
1 small plastic bag (e.g., Ziploc bag) per person tested
 
1 Sample ID Slip per person tested (located at the end of this document)
 

If using a forensic sample as the comparison sample:

1 plain, white envelope or large piece of clean, plain, white paper for each semen-stained sample to be submitted
 
1 plastic bag for each semen-stained sample to be submitted

 

Step-by-Step Instructions

For Forensic Sample

1 Carefully place the semen-stained sample in the plain, white paper envelope or wrap in the clean, plain white paper. Be careful not to touch the stained portion of the sample with your fingers.
 
2. Cut out one Sample ID Slip from the end of this document and fill-in the necessary information.
 
3. Place the completed Sample ID Slip in a plastic bag with the semen-stained sample.  The plastic bag should contain the Sample ID Slip and the semen-stained sample either in an envelope or wrapped in paper.

This completes the forensic sample collection.
Repeat all steps per semen-stained sample being tested.


For Comparison Sample

Depending on the type of comparison sample you submit, the instructions will differ.

If using swabs as the comparison sample:

1 Wait 60 minutes after rinsing mouth before swabbing mouth.
 
2. Cut out one Sample ID Slip from the end of this document and fill-in the necessary information about the person being tested.
 
3. Place the completed Sample ID Slip in a small plastic bag.  This plastic bag will hold the cotton-tip swab after sample collection.
 
4. Place swab inside the mouth between the gums and the inside of the cheek. Rotate the swab 12-15 times. Remove from mouth. DO NOT touch the cotton-tip with your fingers.
 
5. Place swab inside the paper envelope. DO NOT lick the gummed seal.
 
6. Place the paper envelope in the plastic bag with the Sample ID Slip as prepared in Step 3. 

IMPORTANT: Allow the swab to dry for approximately five minutes before placing it in the bag.
 

7. Repeat Steps 4-6 with the second swab.

IMPORTANT: There should be two (2) swab samples per person tested.
 

8. Seal the small plastic bag containing the Sample ID Slip and envelopes so it is air tight (use tape if necessary)

This completes the sample collection.
Repeat all steps per person being tested.

IMPORTANT: For each person tested you should have a small plastic bag containing two envelopes and a Sample ID Slip. Each envelope should contain one swab.


If using hair as the comparison sample:

1.  Complete a Sample ID Slip and Record in Sample Identification Table

Cut-out and complete a Sample ID Slip (located at bottom of the last page) and place in a small plastic bag.  Later, you will need to record the information on Sample ID Slip into the Sample Identification Table (located on Semen Analysis Form).


2. Collect Hairs

Pull six hairs from the head, eyebrow, or underarm, etc.  Use tweezers to grasp the hair slightly above where the hair joins the skin. The hairs can be pulled out with one quick, swift motion. Try to pull thicker hairs if possible. Thicker hairs (i.e., hair from the body or eyebrow) contain more DNA and ensure that we can give you fast and efficient results. If only fine or grey hairs are available, please include 5-8 hairs from that person.

NEVER touch the end of the hair where it was attached tithe skin (the hair root).

The hair root (small clump of skin cells) must be attached to the end of the hair after it is pulled from the body.  Please ensure the hair did not break, but was pulled out completely. It is possible to see the hair root by looking closely at the end of the hair you have pulled out. You may be able to see a tiny round ball of cells at the end of the hair. Remember not to touch this end of the hair.


3. Place Hairs into Plastic Bag

Place the hairs immediately into the small plastic bag with the Sample ID Slip.  Seal the plastic bag. 

Now that the hairs are in the plastic bag, you may be able to see the hair root at the end of the hair without any risk of touching the root.

This completes the sample collection.
Repeat all steps per person being tested.

 

IMPORTANT: For each person tested you should have a small plastic bag containing the hair and a Sample ID Slip.


If using a forensic sample as the comparison sample:

1 Carefully place the sample in the plain, white paper envelope or wrap in the clean, plain white paper. Be careful not to touch the stained portion of the sample with your fingers.
 
2. Cut out one Sample ID Slip from the end of this document and fill-in the necessary information.
 
3. Place the completed Sample ID Slip in a plastic bag with the forensic sample.  The plastic bag should contain the Sample ID Slip and the forensic sample either in an envelope or wrapped in paper.

This completes the forensic sample collection.
Repeat all steps per semen-stained sample being tested.

Cut-out the following slips and use to according the instructions above. You may have left over Sample ID Slips.

Confidentiality and Anonymity Instructions: Use a false name or simply "Man 1" when completing the Sample ID slips and Forensic Infidelity Test Form. Make note of the false names you use.
